Payment can be made by cheque payable to The Royal Australian and New Zealand College of Psychiatrists.

To register for the FPOA/IPA Regional Conference and Exhibition, please complete the registration form and keep a copy for your records. Your registration fee must accompany the completed registration form. Payment can be made by a personal cheque drawn on a New Zealand bank, a New Zealand dollar bank draft, Visa or Mastercard. Registrations will be accepted by fax for those paying by credit card.

Completed registration forms and fees should be sent to The Conference Company.

Confirmation of your registration and a tax invoice will be sent upon receipt of your form and fees.

CANCELLATIONS

Should you need to cancel your conference registration, you may reassign your registration to another person. Please notify The Conference Company.

If you are unable to arrange a replacement, a full refund, less an administration charge of NZ$75 will be made providing notification is received by 11 February 2005. After that date no refunds will be made, but reassigning to another person will be possible up to conference commencement.

If, for reasons beyond the control of the Organising Committee, the conference is cancelled, registration fees will be refunded after deduction of expenses already incurred.
